The short answer
It depends on where the volume went and how fast you want results. Hyaluronic acid fillers replace lost volume right away in areas like the cheeks, temples and chin, and lip filler restores shape to thinning lips. Sculptra works more gradually by helping your body build its own collagen. Many people get the most natural result from a small amount of filler plus Sculptra, placed to balance the whole face.
Understanding your face
Volume loss happens in more than one layer of the face at once. Knowing which layers are involved helps us choose the right product and place it in the right spot.
The deep fat pads under the cheeks and temples deflate with age, which can hollow the temples and flatten the mid-face (Clin Cosmet Investig Dermatol).
The facial skeleton slowly remodels with age, flattening the mid-face bones and letting the chin move back, so the skin above has less support (Clin Cosmet Investig Dermatol).
As estrogen drops after menopause, skin loses collagen and thickness, which can make the face look thinner and less plump (Clinical Interventions in Aging).
Losing weight quickly, whether from a GLP-1 medication, surgery or diet, can cause sunken cheeks and looser skin because the fat under the skin helps keep the face full (Cleveland Clinic).
With age, the upper lip narrows and tucks under as the lower face sags (Johns Hopkins Medicine).
Smoking greatly speeds up how quickly skin ages, and dermatologists recommend daily sunscreen to help prevent premature aging (American Academy of Dermatology).
Know your options
Both restore volume, but in different ways. Hyaluronic acid filler adds volume you can see right away, while Sculptra is a poly-L-lactic acid collagen stimulator that builds fullness gradually over the following weeks (Cleveland Clinic). Many faces do best with both.
Hyaluronic acid filler
Sculptra
How it works
A gel that adds volume where it's placed
Helps your body make more of its own collagen
When you see results
Right away, once swelling settles
Gradually, starting within about a month
How long it lasts
About 6–12 months, and newer products up to 2 years (ASPS)
Can last more than 2 years (Cleveland Clinic)
Best fit
Cheeks, temples, chin and lips; adjustable and dissolvable
Overall fullness and skin quality across the face
Hyaluronic acid filler can be dissolved with an enzyme if needed, which is one reason many first-time patients start there.
Your options
We look at your whole face, not just one spot, and use the least product needed to bring back balance. Here's what each option does best.
Best for: flat cheeks, hollow temples, chin and jawline balance
Hyaluronic acid fillers add volume right away. Deep injections may last up to 12 months, and some newer products up to two years (ASPS).
Best for: thinning lips, lost shape, lip lines
Restores soft shape and hydration to lips that have thinned with age. Lip filler tends to fade faster than filler in less active areas (ASPS), and hyaluronic acid filler can be dissolved if needed (Cleveland Clinic).
Best for: gradual, all-over fullness and skin quality
FDA-approved poly-L-lactic acid that helps your body rebuild collagen. Most people need a series, and results can last more than two years (Cleveland Clinic).
Best for: a natural, refreshed look rather than one 'fixed' feature
Small amounts of filler placed across the cheeks, temples, chin and lips so everything stays in proportion. This is where an experienced injector's eye matters most.
Best for: thin, tired-looking skin over the areas we restore
A regenerative growth-factor treatment, often paired with microneedling, to support skin quality. It doesn't replace lost volume, but it can be a nice add-on to filler or Sculptra.
Best for: volume and skin changes that came with perimenopause or menopause
Hormone therapy has been shown to improve skin thickness, hydration and elasticity (Clinical Interventions in Aging). It isn't a volume treatment, but it can support your skin from the inside.

The inside-out approach
Both can. Estrogen helps skin hold on to collagen and thickness, and skin collagen may drop by as much as 30% in the first five years after menopause (Clinical Interventions in Aging). Rapid weight loss, including on GLP-1 medications, can also leave the face looking gaunt, and Cleveland Clinic suggests losing about one to two pounds per week, eating enough protein and staying hydrated (Cleveland Clinic).
Hormone therapy isn't FDA-approved to treat facial volume loss, and it isn't right for everyone. But if your face changed along with your sleep, mood, cycles or weight, it's worth looking at the whole picture. That's why at Epiphany Clinic, we start with how you feel.
Protect your results
Use a broad-spectrum, SPF 30 or higher, water-resistant sunscreen (AAD). Daily use measurably slows skin aging (Annals of Internal Medicine).
If you're losing weight, aim for about one to two pounds a week with enough protein to protect muscle (Cleveland Clinic).
Smoking greatly speeds up how quickly skin ages (AAD). Quitting helps your skin and your overall health.
Small touch-ups when filler starts to fade, or a Sculptra series to build collagen, keep results natural instead of chasing volume all at once.

Your questions, answered
Should I get filler or Sculptra for volume loss?
Filler gives you visible volume right away in a specific spot, like the cheeks, temples or lips. Sculptra builds collagen gradually for a softer, all-over result. Many people benefit from both. At your complimentary consultation we'll look at your face from every angle and recommend the right mix.
How long does filler for volume loss last?
Deep filler injections may last up to about 12 months, and some newer products up to two years (ASPS). Lip filler usually fades sooner because the lips move so much. Sculptra can last more than two years after a full series.
Will I look overfilled?
Not if it's done thoughtfully. Our goal is to replace what you've lost and keep your face in balance, often using less product than you'd expect. If you ever want hyaluronic acid filler adjusted, it can be dissolved with an enzyme (Cleveland Clinic).
My face looks hollow since I started a GLP-1. What can I do?
You're not alone. Rapid weight loss from any cause can leave the face looking gaunt (Cleveland Clinic). We can help you pace your weight loss through our medical weight loss program and restore volume with filler or Sculptra once your weight is stable.
Is filler safe?
Dermal fillers are FDA-approved medical devices, but they do carry risks, the most serious being injection into a blood vessel (FDA). That's why your injector's training and knowledge of facial anatomy matter. Our injections are done by licensed medical providers.
How much does volume restoration cost in American Fork?
Consultations are complimentary. The cost depends on how many syringes of filler or vials of Sculptra your plan needs, and first-visit member pricing includes $75 off a syringe of filler or a vial of Sculptra. See our price guide for current pricing.
